Pre-Diabetes Community Innovation Project

This project leveraged key clinical indicators supplied through health information exchange (HIE) data to bolster the identification of our pre-diabetes patient population. Early identification of this patient population sought to increase practice awareness and patient engagement to curb disease progression.  The project involved expanding and mapping clinical data elements available through health information exchange with participating practice workflow, patient targeting, care interventions, strengthening community partnerships and population-health metrics.

This project guided by the entire Michigan HealthLink community.  Supported by GMP Network, Livingston Physician Organization (LPO), Olympia Medical, and Oakland Southfield Physicians (OSP).
